Cancer Patient Outreach

To educate Latinx and other community members about cancer and available resources, we have joined a collaborative led by Lazarex Cancer Foundation.

Lazarex Cancer Wellness HUB

Monument Impact is one of four community partners working with the Lazarex Cancer Foundation’s Cancer Wellness HUB, a place-based, community-led, and culturally appropriate model of community engagement for those with cancer and their families.

The Lazarex HUB model focuses on:

Improving health and cancer health outcomes and access to quality cancer care and clinical trials
Providing equitable access to cancer resources; screening for earlier stage diagnosis, no-cost genomic and molecular testing, insurance and financial support, convenient and reliable transportation throughout the cancer journey
Enhancing emotional and financial well-being, patient/resident empowerment
Collaborating with providers, residents and community-based organizations on education and support throughout the cancer continuum for patients, caregivers, and family members

Cancer Diagnosis and Prevention

Our promotores are out in the community talking with residents who have been impacted by cancer or know someone who has cancer. Click here for more resources and information.

Monument Impact’s Role

Our Promotores serve as ‘Hub Ambassadors’ and, with the other community partners, provide a gateway of access to health-underserved communities, identify community leaders for engagement, and guide culturally appropriate messaging and programming. Through our outreach, we will bring patients/residents into the fold of the HUB to facilitate access to needed cancer resources and patient-centered care.
